Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care for older adults in private residences and senior living communities in St. Charles, IL. You will assess, treat, and monitor progress, developing personalized plans while coordinating with an interdisciplinary team.

Qualifications

• Valid Speech Language Pathology license in the state of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• CPR certification and ASHA CCC credential are required.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or communities.
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
