Job summary

Homeside Rehab is seeking a Speech-Language Pathologist (SLP) to join our home health team in Brewster, NY 10509. You will deliver individualized, evidence-based speech therapy in patients' homes with flexible scheduling and a supportive team.

Requirements include an NYS SLP license, 1+ year of relevant experience, strong communication skills, and reliable transportation for home visits. Roles include evaluating patients, creating treatment plans, and collaborating with clinicians to ensure

Qualifications

Active New York State Speech-Language Pathologist license is required.
Minimum 1 year of experience in Home Health, Outpatient, or SNF settings preferred.
Basic computer literacy for documentation and communication.
Strong clinical and interpersonal communication skills.
Ability to work independently in a home health environment.
Reliable transportation for home visits preferred.

Responsibilities

Evaluate patients and develop individualized treatment plans based on patient needs and goals.
Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders.
Maintain accurate and timely documentation of patient care.
Collaborate with the clinical team to ensure coordinated and effective treatment.
Communicate effectively with office staff, patients, and caregivers.
