Qualifications

• 2–3 years of healthcare experience in acute and community settings.
• Licensed MSW (LMSW required; LCSW preferred).
• Knowledge of social, cultural, economic, political, religious, medical, psychological, and legal issues influencing patient and family behavior.

Responsibilities

• Collaborate with the interdisciplinary team to identify patients who would benefit from Social Work Services.
• Complete comprehensive in-home psychosocial evaluations.
• Determine appropriate care levels and develop culturally competent care plans.
• Implement short-term interventions and assist patients/families in goal setting.
• Advocate for patients’ needs to improve access to care.
