Job summary

FOX Rehabilitation is growing in Staten Island, NY, seeking licensed Speech Language Pathologists to deliver one-on-one care to older adults in private residences or Senior Living Communities under the Geriatric House Calls model.

You’ll have autonomy to create your own treatment schedule, deliver evidence-based therapy, and collaborate with an interdisciplinary team, with potential telehealth components to extend access and continuity of care.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in-home or in senior living settings.
• Perform evaluations, develop care plans, and deliver evidence-based therapy.
• Deliver therapy sessions to support wellness, longevity, mobility and independence.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
