Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver in-home and senior living care in Maryland. You will conduct evaluations, create care plans, and provide evidence-based therapy to improve mobility, independence, and quality of life for older adults.

Work includes one-on-one sessions, collaboration with an interdisciplinary team, and opportunities for telehealth when appropriate to extend access and flexibility.

Qualifications

• Valid Speech Language Pathology license in the state of practice.
• Proficiency using electronic medical records (EMR).
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system following regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth or virtual care when appropriate to extend access.
