Job summary

FOX Rehabilitation in Gainesville, VA is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in homes and senior living communities. This role offers autonomy, flexible scheduling, and the opportunity to use telehealth when appropriate to extend access and continuity of care.

You will evaluate, plan, and deliver evidence-based therapy, document in FOX's EMR, and collaborate with an interdisciplinary team to promote independence and quality of life.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Degree from an accredited Speech Language Pathology program.
• Current Certificate of Clinical Competence (ASHA CCC) and CPR certification.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
