Job summary

FOX Rehabilitation in New Palestine, IN is hiring a part-time Speech Language Pathologist to provide one-on-one care in private residences and senior living communities. Clinicians may also incorporate telehealth to extend access and flexibility within the Geriatric House Calls model.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Speech Language Pathology program
• Basic computer and EMR proficiency
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, mobility, and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
