Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care in homes and senior living communities across Fishers, IN and surrounding areas. You will evaluate patients, design care plans, and deliver evidence-based therapy focused on mobility, independence, and quality of life.

Qualifications

• Valid Speech Language Pathology license in the state of practice or eligibility
• Degree from an accredited Speech Language Pathology program
• Basic computer and EMR proficiency
• Current CPR certification
• Current CCC (ASHA) certification

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings
• Perform evaluations, develop individual care plans, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, longevity and independence
• Document in FOX’s EMR according to regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access
