Job summary

FOX Rehabilitation in the United States seeks a Speech Language Pathologist to deliver one-on-one in-home care for older adults across private residences, senior living communities, and memory care settings. You will perform evaluations, develop individualized care plans, and deliver evidence-based therapy while documenting in FOX’s EMR.

Collaborate with PTs, OTs, SLPs, and fitness staff to promote safety and independence.

Qualifications

• Valid Speech Language Pathology license in the state of practice or eligibility to obtain
• Degree from an accredited Speech Language Pathology program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or community settings
• Perform evaluations and develop individualized plans of care and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
