Job summary

FOX Rehabilitation provides in-home and senior living care with a clinician-operated team. SLPs work one-on-one with older adults in homes and communities, performing evaluations, developing individualized care plans, and delivering evidence-based treatment.

Therapists collaborate with an interdisciplinary team and educate patients, caregivers, and staff to promote safety and aging in place. Telehealth options offer flexibility and extended access to care.

Qualifications

• Valid SLP license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Speech Language Pathology program
• Current CPR certification
• ASHA CCC certification

Responsibilities

• Provide one-on-one SLP services to older adults in homes or communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
