Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate care for the patient at home.
• Initiates the plan of care and adapts as patient needs change.
• Administers medications and treatments as prescribed; monitors response.
• Reviews plan of care and education needs; coordinates discharge planning as applicable.
• Communicates changes to supervising physician as needed.
• Document all interventions and outcomes per protocol.
