Job summary

Pathways Healthcare is seeking a per diem registered nurse to provide in-home care in Cranston, RI. You will perform assessments, administer treatments, and develop plans of care in collaboration with physicians and the care team.

Ideal candidates have an active RN license in Rhode Island, 2+ years in geriatrics, strong communication, and knowledge of healthcare regulations. A valid driver's license and reliable vehicle are required. Mileage and a flexible schedule are offered.

Qualifications

Active nursing license in the state of practice.
2+ years nursing experience, geriatrics preferred.
Nursing degree from an accredited institution.
Excellent bedside manners and communication.
Knowledge of healthcare laws and regulations.
Current BLS CPR certification.
Valid driver’s license and own a reliable, insured vehicle.

Responsibilities

Initiate preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed by physicians.
Conduct initial assessments of patients and families to determine home care needs.
Perform thorough physical assessments and gather comprehensive medical histories.
Develop and revise individualized plans of care based on patient status and evolving needs.
Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
Respond to inquiries and concerns from patients, caregivers, and medical teams.
Provide counseling and support to patients and caregivers before, during, and after treatment.
Supervise home health aides as required.
Identify discharge planning needs and proactively implement them as part of the care plan development.
