Qualifications

• Active Nursing license in NH
• 2+ years of nursing experience, geriatrics preferred
• Nursing degree from an accredited institution
• Excellent bedside manners and communication skills
• Must meet healthcare laws and regulations
• Maintain BLS CPR certification
• Valid driver’s license and reliable vehicle

Responsibilities

• Initiate preventive and rehabilitative nursing procedures
• Administer medications and treatments as prescribed by physicians
• Conduct initial patient and family assessments to determine home care needs
• Perform thorough physical assessments and collect medical histories
• Develop and revise plans of care based on patient status
• Collaborate with physicians, nurse practitioners, and teams to create robust Plans of Care
• Address inquiries from patients, caregivers and medical teams
• Provide counseling and support before, during and after treatment
• Supervise home health aides as required
• Identify discharge planning needs and implement them as part of care plans
