Job summary

Help At Home, LLC in Lafayette, IN is seeking a Registered Nurse (RN) Case Manager to deliver comprehensive home health care. You will assess patient needs, develop and monitor the plan of care, coordinate services, and support patients and families to maintain independence at home.

This role requires Indiana RN licensure, one year of clinical nursing experience, and the ability to travel weekly across the territory.

Qualifications

• Graduate of an accredited nursing school.
• One year of clinical nursing experience.
• Active Indiana RN license.
• Experience working with medically fragile clients addressing medical, social and behavioral components of care.
• Regular travel on a weekly basis throughout an assigned territory.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Initiates the plan of care and necessary revisions.
• Coordinates services.
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Gives total patient care as needed; takes on-call duty nights, weekends and holidays, as assigned.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency time frames.
