Homewatch Caregivers of St Cloud Minneapolis St Paul seeks an RN Case Manager to coordinate and deliver in-home nursing care. Responsibilities include clinical assessments, care planning, and acting as a liaison among care teams, with field tasks such as wound care and medication administration.

• Experience conducting comprehensive nursing assessments and developing care plans.
• Strong communication and interpersonal abilities for client and family education.
• Ability to coordinate with providers and supervise unlicensed staff.
• Casual role with regular assessments every 90 days.

Responsibilities

• Perform comprehensive assessments and maintain individualized care plans.

• Complete reauthorizations and maintain accurate documentation.
• Provide health education and support to clients and families.
• Deliver hands-on nursing care (vitals, wound care, medications, discharge planning).
