Professional Case Management is hiring Registered Nurse (RN) Case Managers to perform in-home nursing visits across several South Carolina locations, offering flexible scheduling and strong work-life balance. Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two years nursing experience including one year in home care.
• One year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided.
• Coordinate and participate in client care conferences as needed.
• Assist staffing coordinators regarding staffing within scope of practice.
• Adhere to nursing delegation guidelines and agency policies.
• Participate in performance improvement initiatives including chart reviews.
• Perform supervisory visits as indicated to facilitate problem resolution.
