Professional Case Management is hiring an RN-Case Manager to conduct in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illnesses. You will assess health needs, ensure home safety, manage medications, and coordinate care with caregivers.

The role offers flexible scheduling, strong work-life balance, and comprehensive benefits. Qualifications include an active RN license, two years of nursing experience (one in home care), and CPR certification.

Qualifications

• Graduate of a state approved school of professional registered nursing
• BSN preferred
• Current, unrestricted RN license in the state(s) of practice
• Minimum of two years nursing experience including one year in home care or closely related field
• One year of supervisory and/or case management experience preferred
• Current CPR certification

Responsabilités

• Provide direct care and case management of assigned clients
• Plan, implement, and evaluate care provided
• Coordinate and manage client care conferences as needed
• Serve as a local on-site clinical resource and ensure home care needs are met
• Collaborate with staffing coordinators regarding scheduling and scope of practice
• Participate in interviewing, selection, and ongoing evaluation of clinical personnel
• Review and evaluate clinical documentation for accuracy and completeness
• Support performance improvement initiatives and data collection
