Job summary
Ecumen is seeking a Registered Nurse Case Manager to provide direct care and case management for clients in home and community settings in the St. Cloud area. You will collaborate with physicians and a multidisciplinary team to coordinate comprehensive care plans and ensure patient safety.

Qualifications
- Associate's degree in nursing is required.
- 1 year of nursing experience in post-acute care setting.
- Licensed registered nurse in the state of work.
Responsibilities
- Delivers skilled nursing services to meet client and family needs.
- Performs comprehensive and ongoing client assessments and health histories.
- Collaborates with physicians and other professionals to develop a care plan.
- Develops and evaluates the care plan with clear goals for health and function.
- Communicates physician orders to the agency staff accurately.
- Initiates case conferences to discuss progress and plans for continued care.
- Promotes safety for clients and coworkers.
