Job summary

CareCo Medical is hiring Licensed Practical Nurses for per diem home care visits in New London County, CT. You will provide one-to-one in-home care, documenting visits and coordinating with the care team.

Qualifications

• Must hold a current Connecticut LPN license.
• Valid driver's license is required.
• Reliable transportation and willingness to travel in New London County.
• Strong communication and interpersonal skills.
• Home health experience with Medicare preferred.

Responsibilities

• Perform home visits for patients in a home care setting.
• Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition.
• Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition.
