SM Home Health Care is seeking a Registered Nurse to provide in-home nursing services in Minnesota, under state regulations and agency policies. The role emphasizes compassionate, patient-centered care and documentation accuracy.

You will perform assessments, develop individualized care plans, administer skilled interventions, and collaborate with the care team to achieve optimal client outcomes while maintaining safety and quality standards.

Competenze

• Current, unencumbered RN license in the State of Minnesota.
• Minimum of 1-2 years of nursing experience, preferably in home health, acute care, or community health.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation and valid driver’s license.
• Excellent communication, organizational, and documentation abilities.
• Ability to work independently in diverse home environments.
• Current CPR certification
