Job summary

UnitedHealth Group is hiring registered nurses for home health visits through Caretenders, part of the LHC Group family. You will provide and coordinate nursing care at patients' homes, following physician orders and regulatory guidelines.

Opportunities include full-time, part-time, and PRN positions with a focus on compassionate patient care and collaboration with families and healthcare teams.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Driver's license, vehicle insurance, and reliable transportation.
• Current CPR certification or ability to obtain within 90 days of hire.
• Ability to function in diverse home-care settings with all clients.

Responsibilities

• Provide high-quality nursing care within scope of practice and infection-control standards.
• Coordinate care with the patient and care team from admission to discharge.
• Complete nursing assessments per program and payer requirements.
• Ensure patient eligibility and medical necessity for services.
• Develop and revise care plans with other providers.
• Ensure plan of care frequency/duration matches patient needs and obtain physician approvals.
