Job summary

FOX Rehabilitation is seeking motivated licensed Physical Therapist Assistants to provide in-home and community care in Crown Point, IN. You will work closely with a supervising PT to deliver individualized treatment plans, guide exercises, and help patients regain strength and confidence through FOX’s geriatric house calls model.

Qualifications

• Valid Physical Therapist Assistant license in the state of practice or eligibility to obtain.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by the supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and activities.
• Document visits accurately in the EMR per clinical standards.
• Coordinate with patients, POAs, and referral sources; assist with coverage as needed.
