Job summary

FOX Rehabilitation is growing in Rockford, IL and we’re seeking motivated, licensed Physical Therapist Assistants to deliver hands-on, in-home care to older adults. You will work closely with a supervising Physical Therapist, implementing individualized treatment plans and guiding patients through exercises to improve strength, mobility and independence.

Qualifications

• Licensed Physical Therapist Assistant in the state(s) of practice.
• Graduate of an accredited PTA program.
• Current CPR certification is required.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and activities.
• Document visits accurately in FOX’s EMR system per standards.
• Coordinate with patients, POAs, and community referral sources.
