Job summary

FOX Rehabilitation is expanding in North Augusta, SC, seeking motivated, licensed Physical Therapist Assistants to provide one-on-one, in-home care to older adults through the Geriatric House Calls model. You will work under supervision, delivering individualized treatment plans and guiding patients through exercises to rebuild strength and mobility in community settings.

Qualifications

• Valid Physical Therapist Assistant license in state of practice or eligibility to obtain
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR certification

Responsibilities

• Deliver individualized treatment plans established by supervising Physical Therapist
• Instruct and motivate patients through therapeutic exercises and activities
• Document visits accurately in FOX EMR per clinical standards
• Coordinate with patients, POAs, and community sources as needed
