Job summary

Haven Home Health & Hospice is seeking a Physical Therapist Assistant to join our locally owned care team. You will deliver skilled PT services under supervision, promote independence, and support clients in their own homes.

The role emphasizes collaboration with therapists, accurate documentation, and patient education, with weekend/holiday availability as needed. Missouri PTA license required; one year of PTA experience preferred.

Qualifications

• Current Missouri PTA license in good standing.
• At least one year of PTA experience within the last three years.
• Strong communication and organizational skills.
• Ability to build meaningful relationships with clients, families, and team members.
• Strong clinical judgment and ability to work independently.
• Comfortable with technology and electronic documentation.
• Dependable transportation and a valid driver's license.
• Flexibility to work weekends and holidays as needed.

Responsibilities

• Provide skilled physical therapy services under the supervision of a Physical Therapist.
• Implement individualized plans of care based on physician orders and client needs.
• Monitor client progress and communicate changes with the supervising PT and care team.
• Provide preventive and rehabilitative therapy interventions.
• Educate clients and families on self-management, safety, and physical therapy needs.
• Maintain accurate and timely clinical documentation.
• Collaborate with other Haven clinicians to provide coordinated, client-centered care.
• Participate in department meetings, in-services, clinical outcomes, and performance improvement initiatives.
