Job summary

Haven Home Health & Hospice in Missouri is seeking a dedicated Physical Therapist Assistant (PTA) to work in a home-health setting. You will provide skilled PT services under supervision, following physician orders and individualized care plans to promote safety, independence, and activity for clients at home.

You will collaborate with a multidisciplinary team, document care accurately, and participate in performance improvement initiatives while maintaining Haven’s patient-centered approach.

Qualifications

• Current Missouri PTA license in good standing.
• At least one year of PTA experience within the last three years.
• Strong communication and organizational skills.
• Ability to build meaningful relationships with clients, families, and fellow team members.
• Strong clinical judgment and ability to work independently.
• Comfortable with technology and electronic documentation.
• Dependable transportation and a current driver’s license.
• Flexibility to work weekends and holidays as needed.

Responsibilities

• Monitor client progress and communicate changes with the supervising PT and care team.
• Provide preventive and rehabilitative therapy interventions.

• Collaborate with other Haven clinicians to provide coordinated, client-centered care.
• Participate in department meetings, in-services, clinical outcomes, and performance improvement initiatives.
• Uphold Haven policies, professional standards, and applicable regulatory requirements.
