Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Home Care Physical Therapist Assistant to provide in-home care under the supervision of a licensed PT. This role covers Valley Stream and surrounding areas with flexible scheduling and options for 1099 or W-2 employment.

The PTA must be licensed in New York (or eligible) and will travel to patient homes, performing treatments per the PT plan of care while documenting progress and coordinating with the care team.

Qualifications

• Home health experience helpful but not required
• Demonstrated ability to work independently in patients' homes
• Strong communication with patients and care teams
• Adherence to clinical policies and standards
• Ability to travel within geographic territory

Responsibilities

• Accept and treat patients within your designated geographic area
• Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT’s plan of care
• Monitor and report changes in patient status to the supervising PT in a timely manner
• Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care
• Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT
• Assist the PT in updating care plans based on patient progress and response to treatment
• Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards
• Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT
• Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice
