Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist Assistant to provide in-home, one-on-one care to older adults in Charlottesville, VA. You will work under a supervising Physical Therapist to carry out personalized treatment plans and guide patients through exercises to regain strength and independence.

The role offers meaningful mentorship within a team-based environment, with opportunities to grow clinical skills, caseload, and career through the FOX Geriatric House Calls model.

Qualifications

• Valid Physical Therapist Assistant license in the state of practice.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.

Responsibilities

• Deliver individualized treatment plans established by supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and activities.
• Document visits accurately in FOX’s EMR system.
• Coordinate with patients, POAs, and referral sources for coverage.
