Job summary

Fox Rehabilitation in Bonita Springs, Florida, seeks motivated, licensed Physical Therapist Assistants to provide one-on-one, in-home care to older adults. You will work with a supervising PT, guide exercises, document visits in FOX’s EMR, and collaborate with an interdisciplinary team to tailor plans of care in patients' homes.

Qualifications

• Valid Physical Therapist Assistant license in Florida or eligibility to obtain
• Graduate of an accredited PTA program
• Current CPR certification

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities
• Document visits accurately and on time within FOX’s EMR system
• Stay connected with patients, POAs, and community referral sources, and coordinate coverage when needed
