Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant to provide geriatric home-based therapy in the Jacksonville area. This PRN role offers flexible visits and the opportunity to work with hospice patients in community settings.

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program.
• Active Florida PTA license.
• Two years of clinical experience preferred (not required).

Responsibilities

• Provide skilled physical therapy treatments under the plan of care.
• Help patients improve mobility, function, and independence.
• Communicate effectively with physicians, families, and the care team.
• Complete accurate, timely documentation.
