Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their homes, reporting to the Clinical Manager. You will plan and administer prescribed PT, assess patient strengths, and guide goals with the care team.

The role requires an active PT license and CPR, with experience in a team-based setting. It offers compensation per visit and a supportive Humana network.

Qualifications

• Active Physical Therapy license in good standing.
• CPR certification required.
• Minimum one year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to restore function.
• Assess and document patient strengths to guide treatment goals.
• Develop and implement rehab programs aligned with physician plans.
• Provide direction to PTAs and Home Health Aides with clear care instructions.
• Document observations, interventions, and progress promptly.
• Report progress to physician, Clinical Manager, and family with required summaries.
