Job summary
Senior Care Solutions is seeking a Per Diem Physical Therapist (PT) / Physical Therapist Assistant (PTA) to provide skilled therapy in patients' homes across Indian River County, FL. The role offers flexible scheduling, per-visit compensation, and one-on-one home health care.
Responsibilities include evaluating patients, developing and implementing treatment plans, instructing patients and caregivers, and documenting progress in EMR. Florida license and 1+ year home health experience required.
Qualifications
- Florida license required to practice PT/PTA.
- Minimum 1 year of home health experience preferred.
- Proficient in EMR documentation and workflow.
Responsibilities
- Conduct in-home PT evaluations and plan-of-care reviews.
- Develop and implement individualized treatment plans.
- Instruct patients/caregivers in safe exercise and device use.
- Document progress and modify plans per Medicare guidelines.
- Collaborate with interdisciplinary team to ensure coordinated care.
