Senior Care Solutions is seeking a Per Diem Physical Therapist (PT) or Physical Therapist Assistant (PTA) to provide skilled home health therapy in Titusville and surrounding areas. You will evaluate patients in their homes, create individualized treatment plans, and deliver one-on-one care to improve mobility and function.

Responsibilities include in-home evaluations, implementing plans, teaching exercises to patients and caregivers, and documenting progress in EMR.

Qualifikationen

• Current license in the State of Florida.
• 1+ year of licensed PT/PTA experience, home health preferred.
• Thorough understanding of Medicare documentation requirements.

Aufgaben

• Conduct in-home physical therapy evaluations and reassessments per physician plan of care.
• Develop and implement therapy plans to restore function and mobility.
• Instruct patients/caregivers on safe use of devices and exercises.
• Document patient progress and adjust plans as needed.
• Collaborate with nurses, therapists, and physicians for coordinated care.
• Maintain accurate EMR documentation and ensure compliance.
