Job summary

Qualifications

• Must be licensed as a Physical Therapist in Oregon.
• Graduate of an accredited CAPTE-approved Physical Therapy program.
• Minimum 2 years of PT experience; home health experience preferred.
• Current CPR certification.

Responsibilities

• Develop and implement individualized treatment plans.
• Perform mobility, gait, strength, and ROM evaluations.
• Provide PT services per physician-directed care plans.
• Educate patients and caregivers on exercises and safety.
• Monitor progress and document clinically.
