The Physical Therapist (PT) Home Health plays a vital role in restoring mobility and function for patients receiving care in their homes. This position requires strong clinical skills, problem-solving abilities, and a patient-first mindset. The PT will provide personalized therapy plans, educate patients and caregivers, and ensure compliance with state and federal healthcare regulations. This is a field-based role requiring travel within Eugene and surrounding areas (including Salem when needed).

Education, Certification & Experience
Education: Doctorate or Master's (MS) in Physical Therapy required.
Licensure: Active Physical Therapist License in the state of Oregon.
Certifications: Current CPR/BLS certification required.
Experience
- Minimum one year of experience as a Physical Therapist.
- Home health experience preferred.
Other Requirements
- Valid drivers license with reliable, insured transportation.
- Willingness to travel within Eugene, Salem, and surrounding areas as needed.
