Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one care in patients' homes or senior living communities in St. Clair, MO.

You’ll evaluate, plan, and implement evidence-based treatment to improve mobility and independence. Join a flexible, autonomous role with telehealth options, collaborate with an interdisciplinary team, and help older adults achieve better quality of life through FOX's Geriatric House Calls model.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice.
• Degree from an accredited Occupational Therapy program.
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.

Responsibilities

• Provide one-on-one occupational therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and function.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
