Job summary

FOX Rehabilitation in Bonita Springs, FL is seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences or Senior Living Communities, through our Geriatric House Calls model.

You will evaluate, develop individualized plans of care, and deliver evidence-based therapy to improve mobility, independence, and quality of life, while building lasting patient relationships. Telehealth may be incorporated for flexibility and continuity of care.

Qualifications

• Valid Physical Therapy license in the state of practice or eligible to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions to support mobility, independence, and safety.
• Document in the EMR according to regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients and caregivers to promote aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
