Job summary

FOX Rehabilitation is growing in Bonita Springs, FL, seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities. You’ll evaluate, plan, and treat older adults, supporting mobility, independence, and quality of life through our Geriatric House Calls model.

Enjoy autonomy to manage your own schedule, collaborate with an interdisciplinary team, and optionally add telehealth to extend care.

Qualifications

• Licensed PT with current Florida license or eligibility.
• Doctor of Physical Therapy from an accredited program.
• Strong EMR documentation skills and CPR certification.
• Ability to work independently in home and community settings.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence and safety.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
