Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home PT services, helping clients regain strength, balance, and independence in the familiar environments they cherish.

This part-time/per diem role offers competitive pay ($75–$85/hr) plus mileage reimbursement, flexible scheduling, and support from a dedicated clinical team. In-home evaluations, personalized care plans, and collaboration with a multidisciplinary staff are required.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year of PT experience preferred; new grads welcome.
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile EMR.
