Job summary

AmeriWell Care is seeking a compassionate Physical Therapist to provide in-home therapy across Middlesex County, New Jersey.

You will develop individualized treatment plans and help clients improve mobility, balance, and independence, while collaborating with families and interdisciplinary teams.

Active NJ PT license required; reliable transportation essential; compensation $80–$90 per hour with flexible scheduling and supportive team.

Qualifications

• Active New Jersey Physical Therapy License.
• Experience with IDD populations preferred (Not Necessary).
• Compassionate, patient-centered mindset.
• Reliable transportation for community visits.

Responsibilities

• Provide physical therapy services in home/community settings.
• Develop individualized treatment plans.
• Help individuals improve mobility, strength, balance, and independence.
• Collaborate with families and interdisciplinary teams.
• Complete timely documentation and progress notes.
