Job summary

Always Best Care Senior Services is seeking a Physical Therapist to provide home health care on O‘ahu, HI. The role offers a flexible PRN schedule with competitive compensation per visit.

Responsibilities include evaluations, treatment planning, mobility improvement, caregiver education, and coordination with an interdisciplinary team. Hawaii PT license and at least 1 year of clinical experience are required.

Qualifications

• 1 year clinical experience required.
• Home health experience preferred is a plus.
• Reliable transportation and strong communication are essential.

Responsibilities

• Conduct physical therapy evaluations and reassessments.
• Create individualized treatment plans.
• Address mobility, balance, strength, and safety concerns.
• Provide patient and caregiver education.
• Monitor progress and adjust treatment plans as appropriate.
• Coordinate care with the interdisciplinary team.
• Complete Medicare-compliant documentation.
