Job summary

Phoenix Home Care & Hospice is seeking an RN for Private Duty Care and VA departments to cover in-home visits in Springfield, MO and surrounding areas. This role involves aide oversight visits and routine nursing visits, including medication set ups and maintaining the plan of care.

Qualifications

• Active Registered Nurse license in MO.
• In-Home experience preferred.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver’s license.

Responsibilities

• Provide basic patient care: Taking Vitals, Weekly medication management per physicians orders, Monthly diabetic nails, monthly skin monitoring and Bi yearly general health assessments.
• Opens new cases and obtains all related consents, documentation and deposits in accordance with company policy.
• Observes, records and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician.
• May supervise In Home Aides for Advance Personal Care (APC) Services.
• Provide instruction/ education on medication when needed.
• Follow state approved care plan.
• Attend quarterly Nurse Meetings.
• Good time management, detail oriented and self-starter.
