Qualifications

• Graduate of an accredited nursing program and licensed RN or LPN.
• Current State or Compact RN/LPN license.
• CPR certification.
• TB test and physical examination.
• Reliable transportation and willing to commute to patients' homes.

Responsibilities

• Execute physician prescribed plans of care.
• Complete compliant documentation of care provided.
• Manage and administer prescribed medications and therapies.
• Conduct patient assessments and coordinate care.
• Provide patient education and training to patient and family.
