Job summary

Reliant Medical Group, joined by Optum, seeks an Advanced Practice Clinician (NP or PA) to deliver in-home primary care in Worcester, MA. Join a team dedicated to patient-centered care and system-wide improvements within the Optum network.

The successful candidate will own their patient panel, care for six to eight patients daily, and collaborate with a supportive team while traveling across Metrowest communities. Competitive compensation and benefits offered.

Qualifications

• National Board Certification required.
• Massachusetts Nursing license required.
• Graduate of an Accredited Nurse Practitioner program; Masters preferred.
• BLS certification maintained; other professional certifications as appropriate.

Responsibilities

• Provide in-home primary care to frail and at-risk elderly patients.
• Own and manage your patient panel.
• Care for an average of six to eight patients daily.
• Provide coverage for Metrowest communities.
• Work autonomously within a cohesive team.
