Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver annual risk assessments in home settings or via telehealth, within the NP scope of practice and delegated by the Collaborative Physician.

The role conducts comprehensive health risk assessments, documents visits to NCQA standards, and coordinates with PCPs, care teams, patients, and caregivers to ensure accurate diagnoses and high-quality, home-based care delivery. A valid NP license and board certification are required.

Qualifications

• Active/unrestricted NP license to practice in coverage states.
• Board certification ANCC/AANP/NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid and unencumbered driver’s license with good driving record.
• Outstanding EHR skills.

Responsibilities

• Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions.
• Takes history, examines, determines diagnoses.
• Provides written documentation of patient visit, per NCQA standards
• Takes patient vital signs, as necessary.
• Places case management referrals and communicates with PCP as necessary.
• Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis.
• Performs all clinical duties while observing OSHA Universal Precautions
• Maintains patient confidentiality
• Attends required meetings and in-services and participates in committees, as requested
• Maintains licenses and affiliations and completes other job-related assignments as needed.
