Job summary

Signify Health, part of CVS Health, is seeking a Part Time Physician to perform In-Home Health Evaluations, visiting patients at their homes and helping bridge gaps in care.

You’ll conduct 45-minute visits, review medications and medical history, perform a physical exam, and coordinate with health plans. Travel locally within licensed states is required; virtual visits may be scheduled as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Conduct 45-minute visits with medication and medical history review.
• Perform physical evaluations and, if ordered, additional tests where applicable.
• Conduct virtual visits as needed.
• Travel locally within licensed states as required.
• Deliver services including diagnostic and preventive care as indicated.
