Job summary

Signify Health, Inc. is seeking a Full Time Physician to perform In‑Home Health Evaluations, visiting patients in the comfort of their own homes and bridging gaps in care.

You’ll build trusted relationships and deliver personalized health assessments in a flexible, nationwide program. The role involves 40 hours weekly, with visits that include medication review, history, physical evaluation, and optional diagnostic tests as ordered by the health plan.

Qualifications

• MD/DO license in coverage area(s) (unrestricted).
• Multi-state licenses (preferred).
• Experience in home-based clinical assessments is a plus.

Responsibilities

• Provide in-home health evaluations to patients.
• Perform medication and medical history reviews and physical evaluations.
• Travel locally to visits and conduct virtual visits as needed.
