Responsibilities

• Conduct in-home health evaluations, including history review, physical exam and necessary diagnostics as ordered by the health plan.
• Engage with patients to address health questions and bridge gaps in care.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Collaborate with Signify Health colleagues to deliver Diagnostic & Preventive Services.
