Job summary

Responsibilities

• Provide in-home health evaluations for members.
• Review medication history and medical history during visits.
• Perform physical evaluations and discuss findings with patients.
• Coordinate referrals and information with health plans as needed.
• Travel locally to patient homes and conduct virtual visits if required.
