CenterWell Home Health seeks a Home Health Physical Therapist Assistant to deliver in-home therapy under the supervision of a Physical Therapist. You will implement treatment plans, guide patients toward functional gains, and collaborate with a caring team to support independence and improved quality of life.

Responsibilities include documenting records, participating in in-services, and ensuring safety and infection control.

Qualifications

• Current state license to practice Physical Therapy.
• Read, write, and speak fluent English.
• Valid driver's license and reliable transportation.
• CPR certification and ability to work in patients' homes.

Responsabilités

• Administer the physical therapy treatment plan to patients in their homes.
• Perform non-complex active and passive exercises and modalities as prescribed.
• Document client records and submit weekly to the office.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment failures or supply issues.
• Apply infection control and universal precautions.
